Again, the deadline to complete your physical and paperwork is February 11, 2011.

You are required to print the following paperwork:

#2- Athletic Code of Conduct

#3- Athletic Eligibility Form

#4- Athletic Info Parent Consent for Participation

#5- Steroid Abuse Form
